Funeral Services

• If death has occurred at home, please contact GP to arrange for Medical Certificate (MCCD) to be emailed to the Registrar.
• If death has occurred in the hospital, please arrange for the Bereavement office at the hospital to email the Registrar with the medical certificate.
• Please then make an appointment (2 family members) with the Registrar to register the death. 
• On completion of above, the Registrar will issue a death certificate and also a green form. 
• The green form can be used by the Funeral Director (yourself or a rep from the Rugby Mosque) to book a grave.
